By submitting this form, I/we, being the owner/guardian of this animal, hereby relinquish all claims and ownership of said animal to the non-profit organisation known as Alittlebitiffy Animal Sanctuary and its officers. I acknowledge that said animal will not be returned to me, my family members, or representatives, under any circumstances. Upon completion and submission of this contract, Alittlebitiffy Animal Sanctuary and its officers will have full legal rights and responsibilities for this animal. I hereby state that I have the authority to enter into this agreement as the owner/guardian of this animal. I have, to the best of my knowledge, answered all questions accurately and completely, and have not wilfully misrepresented the health and temperament of this animal.
